Instructions

I would like to invite you to be part of a research project. This project is part of my Master’s Degree in Leadership, at Royal Roads University. This project has been approved by Hockey Saskatchewan, Joel Houseman, Senior Coordinator, Hockey Development – High Performance and Blaine Stork, Senior Coordinator, Hockey Development – Coaching.

The purpose of the research project is to create a conversation with Hockey Saskatchewan stakeholders, including youth and coaches, around youth leadership development. While the outcome and deliverables of the project will ultimately be determined collectively by the stakeholders, the project aims to explore how might Hockey Saskatchewan integrate youth leadership development into its mandate.

As a coach with Hockey Saskatchewan, you are invited to participate and share your experience and perspective of youth leadership development.

This phase of the research project will consist of an anonymous survey and is estimated to last 25 minutes.

The document contains further information about the study and will enable you to make a fully informed decision on whether or not you wish to participate.

Please review the following information:

Participation in this project is not required. If you do choose to participate, you are free to withdraw without prejudice. If you are participating in the survey, you may withdraw prior to the final “DONE” button by simply closing your browser window. The results of this survey will be fully anonymous. Once you press the ‘save’ button on the final page of the survey and submit the survey, there will be no way to retrieve or remove your comments, because there will be no personal identifiers connecting your survey responses to you."
